aspergillus fumigatus keratitis after laser in situ keratomileusis

چکیده
purpose: to report a case of unilateral aspergillus fumigatus interface keratitis after laser in situ keratomileusis (lasik). case report: a 22-year-old woman developed corneal infiltrates located at the flap- stroma interface in her left eye 48 hours after myopic lasik. the infiltration progressed despite administration of topical fortified antibiotics therefore, the flap was lifted and irrigated with antibiotic solution. six days after lasik, visual acuity was counting fingers at 30 cm and a large full-thickness corneal infiltrate was located at the flap-stroma interface. on scanning confocal microscopy, fungal hyphae were observed as high-contrast filaments. the lasik flap was amputated. corneal scraping revealed fungal filaments in smears and culture. the fungus was identified as aspergillus fumigatus. fortified antibiotic drops were discontinued and intensive topical natamycin and systemic ketoconazole were initiated which controlled the keratitis. two months after surgery the eye was quiet and vision improved to 20/200. conclusion: interface fungal infection after lasik is a potentially vision-threatening complication. early lifting of the flap, fungal culture and aggressive antifungal treatment are required to treat this complication. confocal microscopy may be a fast and safe diagnostic tool in determining the presence of fungal hyphae in vivo within the human cornea.

Delayed onset Mycobacterium intracellulare keratitis after laser in situ keratomileusis
RATIONALE Infectious keratitis is a relatively uncommon but potentially sight-threatening complication of laser in situ keratomileusis (LASIK). Mycobacterial keratitis is usually regarded as late onset keratitis among post-LASIK keratitis. There has been no documented case of Mycobacterium intracellulare post-LASIK keratitis of a long-latent period. PATIENT CONCERNS A 36-year-old man was refe...

PURPOSE We report a case of a patient with a history of glomerulonephropathy, not disclosed prior to laser in situ keratomileusis (LASIK), who developed severe postoperative peripheral ulcerative keratitis (PUK) soon after surgery. METHOD Case report. RESULTS Within a week of surgery, the patient, who had no blepharitis or ocular surface disease, also developed diffuse lamellar keratitis (D...

Corneal infection after laser in situ keratomileusis (LASIK) is rare. However, surgical trauma or breakdown of epithelium increases the risk of surface infection. We present the case of a 45 year old woman who developed keratitis due to Mycobacterium chelonae 1 month after LASIK with the Mini-Laser Sight 2000 excimer laser. After an initial improvement following antibiotic therapy the infection...

A 52-year-old man underwent bilateral laser-assisted in situ keratomileusis. Eight months later, he sustained a penetrating corneal injury to the left eye. A dense white infiltrate, unresponsive to antimicrobial therapy, developed in the corneal stroma. Corneal biopsy and eventual penetrating keratoplasty were performed, and both specimens demonstrated fungal elements with branching, septate hy...

trations after oral dosing. The infection was successfully eradicated with preservation of ambulatory vision. This first case of postoperative VRE endophthalmitis is an unfortunate milestone in ophthalmic surgery. Future cases are probably inevitable given the trends in the epidemiology of VRE infection and colonization, especially in patients with compromised immunity. Colonized donor corneal ...
متن کاملCataractogenesis after Repeat Laser in situ Keratomileusis
There has been the unsubstantiated clinical impression that laser refractive surgery accelerates cataract development along with solid experimental data about the cataractogenic effects of excimer laser treatment. We present the first documented case of significant cataract formation in a young myope after repeat excimer laser ablation necessitating phacoemulsification with a posterior chamber ...
